When the GLP-1 ends: keeping the weight off
Every GLP-1 course ends eventually — the prescription finishes, the budget says so, or you and your clinician decide the job is done. What happens next is the least discussed and most important part of the whole journey.
Whether and how to stop is a clinical decision — make it with your prescriber, including how intake is managed as appetite returns. This piece is about the part that is yours either way: the habits that decide if the loss lasts.
Be honest about the headwind
When the medication stops, appetite comes back — often with enthusiasm — while the body is running on fewer calories than it did two stone ago. Studies of people stopping these medications show substantial average regain within a year when nothing else changes. That is not a moral failing; it is what happens when the tool is removed and no structure replaces it.
The people who keep the loss are the ones for whom the medication bought time to build the structure.
What the structure is made of
- Muscle, built in advance. Every kilo of lean mass you protected with protein and lifting during the course is metabolic headroom afterwards. The maintenance phase is won during the loss phase.
- A trained eye for portions. On the medication, small portions happened to you. Off it, they have to be chosen. People who logged and planned while it was easy carry the calibration into the hard part.
- Protein staying front and centre. It is the most filling macronutrient per calorie — the closest legal thing to appetite suppression on a plate.
- A weekly weigh-in with a tripwire. Decide the number in advance — say, half a stone above your finish weight — at which the full plan switches back on. Regain caught at 5 lb is a fortnight's correction; caught at 2 st it is a campaign.
- Training that never went anywhere. If the sessions survived the low-energy months, they barely notice the transition.
The first eight weeks off
Keep everything deliberately boring: same meal structure, same sessions, same tracking — just watch the trend line like a hawk while your appetite finds its new level. Adjust portions gradually as real hunger returns rather than declaring a food festival in week one.
